The Predictors of Recurrence and Surveillance in Superficial Bladder Cancer

We evaluated the prognostic factors that could be used to predict recurrence and the surveillance programs after first transurethral resection of bladder cancer(TURB) in patients with primary superficial transitional cell carcinoma of the bladder.
Materials and Methods:
Between November 1994 and December 2003, 263 patients underwent complete transurethral resection of histologically confirmed superficial transitional cell carcinoma of the bladder. Factors affecting cancer recurrence were assessed using univariate and multivariate analysis.
Median follow-up period was 48.6months(range 24 to 102). For recurrence, urine cytology and intravesical instillation of BCG after first TURB were the only independent and significant predictors. Recurrence rate was significantly higher in patients who had abnormal urine cytology performed previously to first TURB. Intravesical instillation of BCG after initial TURB reduced recurrent rate significantly. As positive predictive value of urine cytology was low, urine cytology could not be substituted for cystoscopy during follow-up.

Although work on prognostic markers is promising, currently none are sufficiently reliable. These data shows that urine cytology and intravesical instillation of BCG are prognostic factors for recurrence. Surveillance schedules are individualized based on the risks of recurrence.
