The clinical presentation of renal cell carcinoma (RCC) is unpredictable due to its various metastatic patterns. Metastasis of RCC to female genitalia is rare, but RCC has been reported to spread to the vagina in patients with asymptomatic primary renal tumors. The authors observed a case of renal cell carcinoma with synchronous vaginal metastasis, which was not detected when the renal mass was initially diagnosed. The patient underwent wide local excision and adjuvant immunotherapy for vaginal metastasis. (Korean J Urol Oncol 2007;5:102-104)
